Application required to receive paid time off under Colorado HFWA
- Employees are responsible for filling out and submitting the application to the leave of absence office at LOAFMLprocess@dhha.org.
- Applications need to be submitted ASAP, but no later than 30 days from the first date of absence.
- Any adjustments to PTO or pay will be completed by 12/31/2020.
- The application can be accessed online here. (The application may automatically download so check your downloads if it doesn't pop up.)
