Application required to receive paid time off under Colorado HFWA

  • Employees are responsible for filling out and submitting the application to the leave of absence office at LOAFMLprocess@dhha.org.
  • Applications need to be submitted ASAP, but no later than 30 days from the first date of absence.
  • Any adjustments to PTO or pay will be completed by 12/31/2020.
  • The application can be accessed online here. (The application may automatically download so check your downloads if it doesn't pop up.)
