1. If you had a positive COVID diagnosis between 7/15/2020 and 10/14/2020 you do not need to do anything. The leave of absence office will review your leave and your pay and send an email notifying you of any additional pay, or PTO owed to you no later than 12/31/2020. You do not need to request this action.
  2. If you had a qualifying absence between 7/15/2020 to 10/14/2020 but did not have a positive COVID diagnosis, you will need to fill out an application for the days / hours you are requesting to have considered. The application is due no later than 11/30/2020 to If your leave is approved and you took the time unpaid, you will be paid back for the approved hours. If you used PTO, the approved hours will be returned to your PTO bank. You will receive an e-mail from the LOA / Payroll teams letting you know the outcome of their review and the amount of pay or PTO that you will receive and when you will receive this.
  3. If you have a qualifying absence as of 10/15/2020 through 12/31/2020 you will need to code your time in API to D_ILL, fill out the application and return it to the email box as soon as possible, but no later than 30 days from the first date of your absence. We will process the application as soon as possible. If due to timing the hours are taken from your PTO bank, or go unpaid, this will be corrected as soon as we are able to process your application.
